2010-2011 Kick-Off Letter and Required Forms

The information included in this letter is designed to provide families with important information related to school activities and requirements for the 2010-11 school year.

We are asking students attending Kick-Off to print their schedules, which will be uploaded to "Student Connect" the day before they are to come to Kick-Off. We will have computers on campus that students will be able to access if they can't print from home.
 
For participation in the 2010-11 Kick-Off, students must submit the following forms that can be downloaded here, ALL FORMS MUST BE COMPLETE AND SIGNED.
  1. Student Enrollment Form (mailed to your home in your kick-off packet) contains current emergency contact information on file. Please review and make corrections as necessary. If you did not receive this form download the black below and return it with all of your other froms at Kick-Off.
  2. Health Information Exchange Consent (Forms are below)
  3. Parent Notification-Health Related Activities (Forms are below)
  4. Student Network Responsibility Contract & Media Release (Forms are below) Zero Tolerance Form – 2 pages – both sides must be signed. (Forms are below)
  5. Parent Notification-HIV/AIDS Instruction (Forms are below) only required to be returned if student is not participating.
Students who can not attend Kick-Off are asked to bring theses forms with them on the first day of school and turn them in at the Counseling or Main offices. A limited number of hard copy packets will be available at the Scripps Ranch Library in early August.
