Athletic Participation Forms
A current Athletic Participation Form and Emergency Card is required to participate or tryout for a sports team at SRHS. The Athletic Participation Form includes a physical exam from a physician or PCA dated May 1, 2011 or later. Once approved, the Athletic Participation Form covers the entire 2010-2011 school year. If your student has not been continuosly enrolled at Scripps Ranch High School since initially entering ninth grade AND they have not been previously cleared for participation at SRHS by the CIF office, please download the Athletic Eligibility Verification Form and submit this completed form with your Athletic Participation Form. Further paperwork may be required before eligibility is granted.

SPORT PHYSICALS
An annual physical examination is required of all students who participate in interscholastic athletics competition. A physical exam in May/June is valid through May/June of the following year. Contact Denny Morrow, the Athletic Director, at 858-621-9020, x1116 if you have any questions.
